Granite Bay Connections, Inc., (GBC) accepts waiver eligible referrals from area agencies/regions, the Bureau of Developmental Services, or guardian. To ensure consideration, an area agency should send a referral packet for review.
Following this review, a "plan" will be developed to meet the needs of the individual consistent with the strengths and mission of the Agency, a "response" will be sent to the referring Region outlining a brief understanding of the individual's needs, the nature and context of anticipated services, along with a proposed budget.
This process, from start to finish can be accomplished in as little as five business days. However, no referral process will be considered complete until a face-to-face interview with the individual has been conducted. Referrals for Programming should include, at a minimum, the following recent materials:
- Neuropsychological or Psychiatric Testing
- Risk and/or Functional Assessment(s)
- Legal or Court Documentation (if applicable)
- Current Person Centered Plan & Current Intrusive Behavior Plan
